Privacy Policy:

*Olivia's Therapeutic Massage will not share or sell any information collected via phone (conversation or text), online (emails, website messages to the company, or online intake forms) , or in office (desk and treatment room conversations and any forms filled out in person) to any other individual or company.

*Stored information will only be collected from Client Intake forms, provided documents from the client, and appointment notes from each appointment. Financial information will not be stored unless the client makes an account with Boulevard and chooses to save card information.

*All personal and health information will be accessible to all staff members and will be used only to provide the safest and best service experience.

*No information collected will be used for personal communication outside of the office.

*Client information and records, including but not limited to financial information, payment history, appointment details, and treatment plans will not be released to anyone other than the client themselves. This must be requested in person and may take 7-10 business days to complete. 

*Any client may opt out of receiving marketing emails or texts by contacting a staff member of Olivia's Therapeutic Massage via phone, email, or online messaging. You may also do this in person at our office.

*Any client has the right to request removal all information and records from our system at any point. Should they choose to do so, this must be done by scheduling a specific time to meet in person at our office.

*Minors are able to receive services with parental consent and a signed waiver. These files can only be copied or removed by the parent on this waiver. An appointment will be required for this request.

*All future changes to this privacy policy will be posted here on this website and client's will be informed via email if or when changes are made.
